Two kinds of coverage

Vision plan or medical insurance?

Many patients have both, and which one applies depends on the reason for your visit.

Vision plans

Usually for routine eye exams and help with glasses or contact lenses.

Medical insurance

Often for eye health problems: red or painful eyes, injuries, infections, sudden changes, or ongoing care for conditions like diabetes or glaucoma.

Before you call

Three things to have ready.

  1. 1

    Your plan name

Both your vision plan and medical insurance, if you have them.

  2. 2

    Member information

Have your card handy. Share member numbers by phone or at your visit, not by email or social media.

  3. 3

    What you need

An exam, glasses, contact lenses, or a specific eye concern. It helps us check the right benefits.

Payment

Clear costs before care.

If any part of your visit isn't covered — an optional test, a specialty contact lens fitting, or myopia management — we'll explain it and the cost before anything is done, so you can decide.

Eyewear

Glasses and frames.

Frames, lenses and eyewear orders are handled by the LensCrafters team in the same store. For eyewear coverage and order questions, they're the best people to ask.

Common questions

Do you take my insurance?

We accept EyeMed and UHA Vision for vision care, and Medicare, HMSA, HMAA, UHA and HWMG for medical eye care. Benefits, referrals and out-of-pocket costs depend on your specific plan and visit. Call (808) 870-2500 to confirm the details before your appointment.

What's the difference between vision and medical insurance?

Vision plans usually cover routine exams, glasses and contact lenses. Medical insurance may apply when a visit is for an eye health problem, such as an infection, injury, or monitoring a condition like diabetes or glaucoma.

Is a contact lens fitting covered?

A contact lens fitting is often separate from the eye exam, and coverage varies by plan. We'll help you understand what applies before your visit.
